Home Safety & Fall Prevention (HARP Certified Program)

Falls are one of the leading causes of injury for older adults, but most can be prevented. Our Home Safety and Fall Prevention Program is led by occupational therapists trained in the Home Hazard Removal Program (HARP) — an evidence-based intervention designed to reduce fall risks right where they happen: at home.

Through 2–5 personalized sessions, we help older adults identify and correct fall hazards in their environment and daily routines.

Our process includes:

Comprehensive Assessment: We evaluate the person, their home environment, and how they move through daily tasks.

Home Hazard Removal Plan: Together, we create a plan that targets safety risks and recommends practical solutions.

Remediation: We guide and assist with changes such as installing grab bars, rearranging furniture, or introducing adaptive equipment.

Self-Management Strategies: We teach simple ways to improve awareness of safety and reduce behavioral risks.

Cognitive Health & Memory Support (BCAT® Trained Program)

Our therapists are specially trained in the BCAT® Approach, a leading evidence-based program for assessing and addressing memory and other cognitive challenges.

The BCAT® (Brief Cognitive Assessment Tool) is used worldwide by healthcare professionals and is designed to help individuals maintain or improve their cognitive abilities, even when facing early or moderate cognitive decline.

The BCAT® system includes:
